Please do not hesitate to speak with us if you have any questions or concerns about our billing policies or your treatment. You will be asked to sign a Patient Registration Form that indicates your understanding and acceptance of these policies. Please keep the form for your records. We are HIPAA compliant and follow the most restrictive rules of confidentiality.

You may make a payment on your account via PayPal by clicking the link to the left. Please indicate the amount of your payment and complete the transaction

Your First Visit
Unless we have a contract with your insurance coverage that requires another arrangement, payment for your first appointment is required at the time of the appointment. The payment will be applied to your deductible, payments or co-payments that are due subsequently.

 

Initial Assessment: $185
45-minute Individual Session: $125
45-minute Family Session:
$145
Group Session:
$80

 

Subsequent Payments/Co-Payments
Further payments or co-payments are due at the time of service. A $5.00 surcharge will be added to your account if fees are not paid on the date of service. Payments for evaluations, personal payments or co-payments are expected on the date of service.

Cancelled or No Show Appointments
Fees will be charged automatically to your account for all late cancellations and no show appointments at the following rates:

No Show: $85
Group No Show: $60
Late Cancellation: (24 hours or less)
$65

No further appointments will be scheduled until the balance is paid in full. If missed appointments persist, therapy will be discontinued. 

Insurance Billing
We will bill your insurance company directly if you provide us with all the necessary insurance information. You remain responsible for the cost of services and for assuring that insurance reimbursements are timely and accurate. Please keep our office informed of any changes in your insurance.

We provide assistance in facilitating your receipt of insurance benefits to which you are entitled. We file insurance claims promptly and provide timely treatment plans as may be required. It is important for you to understand exactly what your insurance benefits for mental health treatment are so that you and your therapist can plan treatment accordingly. Insurance companies may require you to authorize us to provide a clinical diagnosis.

If your insurance company requires a deductible, Positive Pathways will accept a payment of $85.00 for each session until that deductible is met. This payment is also due at the time of each session.

Treatment Summaries
Additional information such as periodic treatment plans or treatment summaries may be required. This information will become part of the insurance company files.

Charges for Telephone Calls
You will not be charged for brief phone contacts of ten (10) minutes or less or for brief contacts with other support services on your behalf such as schools or other treatment providers, etc. Longer phone conversations, travel time, letter writing, and report preparations are billed on a prorated basis. Higher legal fees are charged if our participation is required in legal proceedings. Click here to view our fee chart.
